If you suspect that your patient has suffered an adverse drug
reaction, please consider sending us a Yellow Card, especially if the
reaction is serious or it involves
a black triangle drug, or is a drug
which has been used in a child.
Reports can be sent directly to us using the printed Yellow Cards in the British National Formulary.
We can also send you a supply of pre-paid cards or you may download
